Use of the Burn Permit Application
Anything submitted using the application form is a matter of public record and is eligible for release in a public records request. We also provide the information on request from the Arizona Department of Environmental Quality.

Patient care reports
Patient care reports may be released in their entirety under the following circumstances:
- They are provided in their entirety to our base hospital (as of 1-30-2023, this is Canyon Vista Medical Center) for Quality Assurance and Quality Improvement purposes.
- They are provided in their entirety to an agency transporting you and/or the emergency room to ensure continuity of care.
- They are provided in their entirety to our billing company and your insurance company for billing purposes.
- You are requesting your own patient care reports and have provided a written request and photo ID. You will have to appear in person with all documentation.
- You are the surviving spouse of a deceased patient and have provided an original death certificate, written request or photo ID. You will have to appear in person with all documentation.
- You have a valid Healthcare Power of Attorney, the patient is unable to make their own decisions, you have provided a written request and photo ID. You will also have to appear in person with all documentation.
- Unless otherwise specified in the document, a Healthcare Power of Attorney, Advanced Directives or Living Will are only valid if the patient is unable to make their own decisions. All of these documents become invalid upon the death of the individual.
- You have a valid court order. In this care, you will have to appear in person with all documentation. We may send the order to the County Attorney’s office for verification or clarification, which can delay the production of the documents.
Use of patient care reports for case studies
We are allowed to use patient care reports as case studies for training purposes provided that all personally identifiable information is removed. This includes your name, address, social security number, driver’s license number, physician, insurance information, employer, photos, et cetera. We are only allowed to use your age, sex, medical history, medications, allergies and the narrative text from the patient care report.
Changes to this privacy policy
We have the discretion to update this privacy policy at any time. When we do, we will revise the updated date at the bottom of this page. We encourage Users to frequently check this page for any changes to stay informed about how we are helping to protect the personal information we collect. You acknowledge and agree that it is your responsibility to review this privacy policy periodically and become aware of modifications.
